Accessing Disability Resources
If you have a disability and would like to receive accommodations at UMD, please follow these steps.
1. Download and complete the Request for Accommodations Form. Print a copy for DR (and one for your record if you would like one).
2. Attach a copy of your disability documentation. If you do not have your documentation, you will be asked to send for it at your appointment. Bring the name and address of the licensed professional who provided the documentation to facilitate this.
3. Hand-carry or send your Request for Accommodations Form and your documentation to KSC 258 and schedule your appointment with a disability specialist.
4. If you are currently attending classes at UMD, bring your course schedule and a syllabus for each class in which you want accommodations to your DR appointment.
5. You may also schedule your appointment in person at KSC 258, by calling 218-726-6130, or at the Multicultural Information Desk. You will be asked to complete the Request for Accommodations Form before your first appointment.
