Test Accommodation
Student Responsibilities
1. Download, complete, and print Request for Accommodations Form. Bring it and your documentation to KSC 258.
2. Make an appointment with a Disability Specialist to discuss needed accommodation and get your Test Accommodation forms.
3. Fill out top section of each Test Accommodation form.
4. The disability specialist will email a Letter of Accommodation (LOA) to you. Print a copy for each of your professors/instructors.
5. Meet with each professor, taking LOA and Test Accommodation Form with you. Discuss your needed accommodations with each professor. Ask him/her to complete and sign the accommodation forms.
6. Bring the completed accommodation form to KSC 254 or KSC 258 at least three working days prior to your first exam.
7. If you have any difficulty getting your accommodation completed, contact DR immediately.
8. Arrive at DR 5-10 minutes before your exam. Accommodated time begins when your exam is scheduled. Students who are tardy will have those minutes subtracted from their total time. After 20 minutes, a student is considered a "no-show" and will be advised to talk to their instructor about rescheduling the exam.
9. All exam changes, including rescheduled exams, must be submitted in writing on a Test Change form. Any change made for a personal reason, must have the instructor's signature.
10. Only the materials specified by the instructor will be allowed in the testing room. All other materials (jackets, hats, bags, cell phones, calculators, etc.) must be left in the DR student employee office. DR is not responsible for lost or stolen items.
11. All students are expected to abide by the University of Minnesota Regents Conduct Code,
Section V. Disciplinary Offenses, Subd.1.
Subd. 1. Scholastic Dishonesty. Scholastic dishonesty means plagiarizing; cheating on assignments or examinations; engaging in unauthorized collaboration on academic work; taking, acquiring, or using test materials without faculty permission; submitting false or incomplete records of academic achievement; acting alone or in cooperation with another to falsify records or to obtain dishonestly grades, honors, awards, or professional endorsement; altering, forging, or misusing a University academic record; or fabricating or falsifying data, research procedures, or data analysis.

All testing in DR will be monitored.
