Background Checks for Students

UMD requires background checks for students who are placed in student teaching, clinicals and other internship experiences at off-campus sites.

The following documents should be completed and approved the semester before students begin their placements:

• Background Check Consent Form
• Personal Disclosure Statement
• Summary of Rights under the Fair Credit Reporting Act
• Driving Record Check Request Form - needed for those who will need to drive University vehicles
