Examination forms

The UMD Graduate School office issues examination forms for Duluth students. These forms are needed at the final oral exam. For students following Plan A (with thesis), there are two forms, one stating readiness of the thesis and the other reporting the result of the exam. Students following Plan B (three papers) do not need the thesis-readiness form.

at least two weeks prior to the examination to request the forms. Requests should include

• the student's name
• the student's ID number
• the date the exam will be held
• the committee members
• the committee chair

Following the exam the paperwork should be returned to the UMD Graduate School office for processing.

Note that:

• Students must have an approved Degree Program on file prior to requesting the examination paperwork.
• Committee changes must be done before the exam takes place so that the examining committee listed on the examination form is correct.
