The Faculty Information Record (FIR) in Microsoft Word (.doc) format

To be completed by all faculty every year. The FIR for the year 2007 covers the period 1 January - 31 December 2007, and is due to the Dean's office by 4:30 p.m. on Friday 29 February 2008. NOTE: The FIR format was changed as of December 2005. Please note the new guidelines below.

Don't fill out the Faculty Information Record (FIR) without consulting the Guidelines at every step. Important information on how to fill out the new FIR is included in these updated guidelines.


"Blue Tally Sheet"

Gets its name from being printed on blue paper. The sheet for compiling information for the year 2007 is due along with the FIR by 4:30 p.m. on Friday 29 February 2008.


Promotion &/or Tenure Consideration Form in Acrobat (.pdf) format

Required of all probationary faculty and strongly recommended for any tenured faculty member who is not a full professor. Due by 30 September of each year.

Appraisals of Probationary Faculty - Form 25 (pdf) Form 25 (doc)
