- Status Change Form (Supervisors Use Only)
Supervisors use this form to send employee status changes to the office for processing. (inactive, term, title change, etc.)
- Key Shop Access Forms
Use this link to access the key shop web site where you can request
keys, proximity cards, special event access (for doors with proximity
readers), or link to work order sites. A pin number will be required
to send any requests.
- Work Request Form
Please do not use this form to report situations that could result
in fire, major property damage, saftey hazards, or jeopardize campus
security. Only use this requesting method for routine basic maintenance
needs. Examples: burned out light bulbs, light switch repairs, inoperable
window crank, broken window shade, etc. For immediate assistance or

- Address Change Form
We are unable to determine who has been added or deleted from faculty/staff
without notification. Please use this form for campus address changes
when faculty/staff changes occur to facilitate mail delivery.
- Department Information Form
There have been many changes to colleges, departments and buildings
in the past few years. Each department is required to have a form on
file in the key shop. Any requests for key shop service from a department
without a report on file will be sent to the appropriate dean or department
head for approval.
- Keyshop Web Access Request
This form is used to authorize individuals to request keys, proximity
cards and special card access event programming for the areas for which
you are responsible.
