To Enroll:
UMD Health Services is a participant in the Minnesota Family Planning Program. To enroll, students need to complete a short form. Upon completing the form, students are given a 30-day 'pre'enrollment term.
To obtain an official Program card and this free coverage for one full year, bring the following documentation to your Health Services appointment:
• Student ID
• Proof of citizenship: copy of birth certificate or passport
• Proof of income: 30 consecutive days of pay stubs (excluding work study) to show income meets limits.
Please note: Students that are currently receiving birth control and are registered patients at UMD Health Services can fill out the application and bring it in to receive their usual prescription without first seeing a medical provider. Patients new to UMD Health Services will need to fill out the application and make an appointment with a medical provider as part of the benefits process.
