REQUIRED FORMS All three forms are required to be filled out for participation
in the trip. The PCUSA General Release, Indemnification Agreement and Authorization
for Medical Treatment form also requires a witness to your signature. The check for the registration fee of $350.00 should be made out to United Campus Ministry.
On the Memo line put New Orleans Trip. The registration fee is non-refundable. Spaces are allotted on a first-come, first serve basis; registration is confirmed once all three
forms are completed and returned with the registration fee. The check and the three forms can
be sent or brought to Monte Gomke, UMD Housing & Residence Life, Lake Superior Hall 149,
513 Niagara Court, Duluth MN 55812-3046 by February 1, 2008 to confirm a space for the trip. FORMS
PLEDGE FORM This pledge form can be used to raise money for yourself for the trip. You can contact your parents, relatives, friends, etc. and ask them to support your trip to New Orleans to help the residents rebuild their homes. The money you raise will be for your own use. It will not go into a pot and be split among all the participants.
