Student Data Sheet Form

This form is distributed at Internship meetings held at the beginning of fall and spring semester. It is the first step in planning to become enrolled in one of the LSBE Internship courses. If you have not submitted a completed copy of this form, please print a copy from this web site, fill it out printing neatly to avoid errors, and drop off the form at the LSBE Internship Office in Room SBE 45. Submitting a student data form does not automatically ensure that you will be offered an internship. Successful placement is dependant upon the time and effort you as a student are willing to spend. The Internship Office will assist in identifying potential opportunities, provide feedback and suggestions on job application material, and help you determine what type of experience may be best for you.

Student Learning Objectives Form

 This document is part of the assignments for the LSBE Internship Program. After opening the document, type your goal statements which include both an objective to be accomplished, and a “plan of action” as to how you will achieve each goal. Also add your name as a header in the upper right-hand corner of the page.

After you have edited the form to fit all the text onto one page, and the text you add is not bolded, print a copy of the document. Drop off this copy at the LSBE Internship Office, Room SBE 45, and review your work with the Internship Director. You may be asked to revise your work to meet the requirements of this assignment.
