Medication Therapy Management (MTM) is a new service offered
at no cost to eligible UPlan participants. MTM allows enrollees
to become more active in medication therapy decisions and
can result in improved health and savings on medication
copays.
MTM seeks to reduce medication therapy problems by providing
an individualized patient care plan. The more medications
one takes, the higher the chances of having medication therapy
problems. MTM can help reduce that risk.
How does it work?
A specially trained pharmacist will work with you and your
physician to complete an initial health assessment and create
a personalized medication record. Next, a medication action
plan will be developed to ensure that the medications are
necessary, safe, effective, and convenient. Regular follow-up
visits ensure you continue to receive the most effective
and cost-efficient medications available.
Who are eligible to participate?
- UPlan participants, (active employees, dependents, early
retirees, or disabled participants) of the University
of Minnesota Duluth; AND
- Must be currently using four or more prescription medications
and have at least two chronic health conditions; OR
- Have a referral from your primary care provider

Medication Therapy Management Clinic is located in Room
103, Kirby Student Center. Free parking is available near
the Kirby Student Center.
