Copyright Permission Requests

Use the form below as many times as you need to enter citations. Each time you hit "Submit Record" you will be adding a permission request. To submit additional citations for the same course packet, click the "back" button on your browser, change only the portion of information needed for the next citation and click "Submit Record" again.

Complete bibliographic information for each item submitted is required in order to process your requests.
Publishers will not respond to incomplete requests.

Please note:

ANTHOLOGIES: If you are requesting permission for material from an anthology, it is likely that the publisher of the anthology does not control the rights to the material. YOU must check for a citation of the original source, either from the acknowledgment page or at the beginning or end of the material used.

RIGHTSHOLDER: Please review your sources carefully to find who actually holds the copyright. Don't assume that it is the publisher or the author. Look for the copyright symbol (letter c in a circle) with the date and the name of the company or individual.

You may bring your originals for duplicating once you have completed this request form. We will not make the copies
until all citations have been processed, fees are determined and we have a final okay from you to proceed.
