Agency Description: HDC’s mission is to lead our communities by providing integrated, culturally respectful mental health and addiction services that foster hope, self-determination, and recovery. Our focus on serving those most in need improves the quality of life for all. HDC serves those living with severe mental health issues of all ages.  The Tact program (transitional assertive community treatment) specifically works with young adults ages 18-25 who have severe thought disorders, OCD, Borderline Personality, and Schizophrenia to name a few.  Tact provides intense case management to clients to prevent hospitalization stays. It offers case management, psychiatry services, nursing, and financial assistance. This program is an evidence based practice proven to be supportive for community living.   
Description of Field Activities

Tact is a collaborate team consisting of HDC, St. Louis County, State, and Lifehouse staff. Students will work with young adults who live with severe mental health disorders; schizophrenia, borderline personality, and obsessive compulsive disorder. The clients work on a range of issues from medication management, employment, housing, mental health management, parenting, and family dynamics.
