This is a private non-profit hospital. The mission is to improve the
health of the communities we serve. We are the regional provider of choice
through innovation, diversity, leadership, and medical excellence. We provide
a positive, caring, environment which ensures the highest quality of life,
customer service satisfaction and employee excellence. Above all else-we
do the right thing. The populations we serve are members of our communities
who are admitted as patients. Ages range from birth through geriatric. |
| Description of Field Activities |
The student will be exposed to several opportunities. These include developing
a needs assessment for the community resources we refer hospital patients
to. The goal of the needs assessment is to identify opportunities to change
or enhance our current way we do business. This will also involve planning,
organizing, and implementation of a satisfaction survey for these identified
community referral sources. As a Joint Commission requirement our department
is required to have a resource (choice of vendor) book available to make
informed choices for post continuum of care. Updating the resource library
and accessing the community providers for current information for facilitation
of comprehensive patient care/choice. The care management team meets every
other week. This program is 6 months old. The student will have a role
in evaluating our progress with the team. There will be time to assist
in the planning of electronic medical record development for the team.
Participating in performance improvement program process and quality program
assessment will be provided. The student will attend at least 2 manager/supervisor
meetings. Plan community education program for community members on Advance
Directives. May have a role in assisting with advances directives for the
people who come into the facility for the completion of this document.
There will be a patient case load for patients in the hospital completing
assessments and doing discharge planning. |
