Hiring department (supervisor or payroll representative) must fill
out the form for their employee/s if they:
Hiring department does not need to fill out this form if it is a transfer from one department to another with employee already on 75% or more appointment from previous department and there is no break in service.
Transferred to 75-100 % appointment
Temporary appointment of 75-100% lasting three months or more
50-74% appointment lastting three months or longer
