The Bodily Injury/Property Damage Incident Report (PDF)
is used to report personal property damage and all non-work related bodily injury incidents that occurs on campus. For
damage to University property, contact the Office of Risk Management
and Insurance at 612-625-0062.
Note: For work-related injuries, employees need to complete the Employee
Incident Report (PDF).

Complete the Bodily Injury/Property Damage Incident Report (PDF)
for non-work related incidents. Work-related incidents should be reported
on the Employee Incident Report (PDF).
