SLIPS, TRIPS, AND FALL INCIDENTS
IF INJURY/ILLNESS IS AN EMERGENCY,
DIAL 911 FOR IMMEDIATE ASSISTANCE |
All incidents MUST BE REPORTED IMMEDIATELY to UMD Campus Police (726-7000) and the UMD Facilities Management (726-8262). This can be done by the person who has fallen, if s/he is able, or by someone in the area of the fall or an employee's department.
Additionally, the following paperwork should be completed for all slip, trip and fall incidents:
FOR EMPLOYEES (during work hours):Employee Incident Report (workers' compensation), follow Step 2 at http://www.d.umn.edu/umdhr/WorkComp/directions.htmFOR OTHERS: (1) students not employed at the University of Minnesota, (2) visitors, and (3) employees during non-work hours:*Fill out Personal Injury Accident Report from the web or pick up a form at UMD HR, 255 DAdB.
* Follow the instructions on the form.
