REQUEST FORM FOR DEPARTMENTAL AED AWARENESS TRAINING

*Required Field- need to be filled out before sending

Department's Responsibilities:

  • Reserve a room
  • Make arrangements for audiovisual equipment, TV/VCR
  • Prepare a list of employees attending the training with their Empl ID and a space for their initials or signatures and make available to the Trainer on the day of the training.
