EMPLOYEE MEDICAL FMLA LEAVE
GUIDELINES FOR SUPERVISIOR:
1. Promptly notify UMD Department of Human Resources of the need for FMLA
leave if the employee has not done so already (or if you are unsure whether
the employee has done so). HR will send out the required paperwork, copying
you and the Departmental Liaison for your area.
2. The Civil Service Personnel Action Document - PDF
/ WORD document
must be completed by the supervisor noting that the leave is a FMLA covered
leave and the date of the provisional FMLA letter. The PAD document should
be sent to UMD Department of Human Resources, 255 DAdB.
3. Complete the Family
and Medical Leave Record. Up to twelve (12) weeks paid/unpaid
time is covered under the FMLA every fiscal year.
NOTE: If the employee has a C-section or pre- or post-delivery complications,
and provides the appropriate documentation from their doctor (the FMLA
certification form), she will be allowed to use sick leave to cover the
time the doctor states she is disabled.
