OUTSTANDING SERVICE AWARDS - Outstanding Service Awards (Continuous Service and Project Award) have been developed to recognize the outstanding contributions of employees in the following job classifications: (1) Civil Service (non-bargaining) employees; (2) AFSCME employees; (3) TEAMSTER employees; and (4) Academic Professional and Administrative (P&A) employees who do not hold faculty rank. Employees holding faculty rank are eligible for other monetary awards. Flu Shot Clinic schedule is also available at http://www.d.umn.edu/umdhr/healthfair/2008/ OPEN ENROLLMENT runs from November 1-December 1, 2008 and takes effect January 1, 2009. You will receive the Open Enrollment booklet from UofM Employee Benefits soon. EMPLOYEE HEALTH AND WELLNESS CENTER was created to help UPlan members
and their dependents become more healthy. It offers a variety of services
to help you obtain and maintain good health. Services are low-cost or
free to eligible UPlan members. EHWC consists of QuickCare Clinic, Medication
Therapy Management Clinic and Wellness Coaching. QuickCare Clinic is a clinic for UMD employees, spouses/same-sex domestic partners, and dependent children. An experienced physician assistant provides basic services such as treating ear pain and strep, to immunizations. Co-pay is $5.00. No appointments needed and visits usually take about 15 minutes. There is no copayment for immunizations. http://www.d.umn.edu/quickcare/ Medication Therapy Management Clinic (MTM) is a service offered at no cost to eligible UPlan participants. MTM helps creates a partnership between you, your physician and a trained pharmacist, who work together to ensure all of your medications are necessary, safe, effective and convenient. http://www.d.umn.edu/mtm/ The Wellness Coach can help motivate and encourage eligible UPlan members to better health. Your health coach provides unbiased, up-to-date information to assist you with the development of a personalized wellness plan to effectively change behavior. http://www.d.umn.edu/healthcoach/ UMD TOBACCO/SMOKING CESSATION PROGRAM (TSCP) - If you’re looking
to quit, the UMD Tobacco/Smoking Cessation program is here to help. Our
team of health professionals will assist you in creating a customized
plan to become tobacco free. We will support you along the way with guidance
from a qualified health educator, prescriptions for tobacco cessation
medications, and medication counseling from a qualified pharmacist. The
counseling program is free for UMD faculty, staff, and their families.
EMPLOYEE ASSISTANCE PROGRAM - St. Luke’s EAP provides assessment
and referral services to promote the health of employees and their immediate
family members. Using EAP both as a preventative service and crisis intervention
service for personal and job-related problems is encouraged. Available
24-hours a day, 365 days a year, to make an appointment or for information:
1.218.249.7077 or 1-888-355-8495. UMD COOKBOOK - Consider submitting a recipe for the UMD Cookbook. UMD HIP (Health Improvement Program) is one of UMD Employee Wellness
Program's offerings in collaboration with the Employee Health & Wellness
Center/Wellness Coaching. It is free and designed to help faculty and
staff who are: looking for a safe and effective health behavior change
program, striving to reach their optimal health, and looking to improve
nutrition and exercise habits as well as stress management and overall
wellness.
