The forms listed are suggested formats for programs and units use in submitting pieces that together will make up the program assessment plan. The forms can be adapted or modified if needed to represent the program's approaach to assessment.
Step 1. Write program outcomes.
Step 2. Map program outcomes to institutional outcomes.
Step 3. Map program courses/components to program outcomes.
Step 4. Determine how and when outcomes will be measured.
Step 5. Conduct assessments, analyze the data and report results.
