THIS FORM MAY BE APPROPRIATE FOR INTERNSHIPS, PRACTICUMS, FIELD EXPERIENCES, OR OTHER "REAL WORLD" EXPERIENCES. This form can be used in conjunction with the Internship Questionnaire (below). The Questionnaire can be copied from this site and used.

NOTE: "INSTRUCTOR" means the person from UMD who oversees your internship experience.

1. The instructor did as much as possible to make the experience valuable to me.

2. The instructor motivated me to do my best.

3. The instructor explained what work and behavior was expected of me in the practicum setting.

4. The instructor provided me with a description of the skills that I was supposed to be proficient at as a result of the experience.

7. The instructor helped me in my relations with other people in the setting.

8. The instructor conferred with me before site visits about issues and concerns in the setting.

9. The instructor observed me and helped me solve difficulties that I encountered on the site.
