National Health Service Corps Scholarship Program

Service-obligated scholarships for full-time students of allopathic (M.D.) and osteopathic (D.O.) medicine dentistry (3rd and 4th year), family nurse practitioner, nurse midwifery, and primary care physician assistants. Other health disciplines may be included after 2002 if needed by National Health Service Corps. Scholarship pays tuition and required fees for the year to the school, pays a monthly stipend to student ($1,065 for 12 months of the 2002-2003 school year) and a single annual payment to the student to cover the cost of all other reasonable educational expenses (books, supplies, equipment, uniforms, clinical travel, etc.) based on costs by school and class year. Awards are limited to U.S. citizens due to potential for Federal employment during service. Applicant must be enrolled in a U.S. accredited school. Each year of support incurs 1 year of service. A 2-year minimum service commitment is required. A maximum of 4 years of support may be awarded. Service is primarily a nonfederal salaried or contracted employee of a public or private non-profit entity, providing full-time primary health services. Services may also be in a fee for service private practice setting, or a Federal employee in Federal health systems such as the Indian Health Service, Immigration Health Service or Bureau of Prisons. Service sites are selected by the scholarship recipient from the list of high priority sites in HPSAs identified by the National Health Service Corps. Physicians receive their site list at the beginning of their last year of residency training; nurse practitioners, nurse midwives, and physicians assistants receive their site list within 6 months of their graduation. Service sites are limited to the United States, territories, and successor States to the Trust Territories, the Republic of Marshall Islands, Micronesia, and Palau. Deferments of service are granted physicians to complete residencies in family practice, internal medicine, pediatrics, psychiatry, and OB/GYN.

Marshall H. and Nellie Alworth Scholarships

Awards up to $4,600/year. Students from the following Northern Minnesota counties are eligible to apply for the Alworth Scholarship:

Aitkin, Beltrami, Carlton, Cass, Clearwater, Cook, Crow Wing, Hubbard, Itasca, Kanabec, Koochiching, Lake, Lake of the Woods,

Pine and St. Louis. Applicant must have demonstrated qualities of character, perseverance and ambition, in addition to high scholastic

standing. Need must be demonstrated. To qualify you must do one of two things: 1) Complete the FAFSA, and send a copy of the Student

Aid Report. While the deadline for receiving the Alworth Scholarship applications remains January 15, the Student Aid Report may

be sent later, but before March 1; 2) Applicants and their parents can use their 2000 IRS form 1040 to affirm their financial need. If

their family's tax return has not been completed by January 15, the tax form can be sent to the Alworth Office after the deadline, but

before March 1. If you wish to be considered for it in subsequent years, you should request a renewal letter prior to March 1. January

15 is the deadline for initial applications.

Armed Forces Health Professions Scholarship - Army, Navy, Air Force, MN National Guard

Commission in one of the branches of the U. S. Armed Forces. Must not be over the age of 36 unless prior military service. Tuition,

fees, books and supplies paid by the armed service in addition to a stipend of $938/month. One year of service for each year of support

at a minimum of three years service after graduation. Summertime duty commitment is likely while you are in medical school. Contact

your local recruiter. There is an early deadline on these programs generally in April. Competition for these programs is high. A limited

Charles M. and Ruth H. Bagley Scholarship

Scholarships in the amount of approximately $1,500 will be awarded to three or four second year medical students who are

outstanding in the area of clinical medicine. Recipients will be chosen by a special selection committee.

Bevan W. Bunker, M.D. Medical Scholarship

This award is up to $2500scholarship/grant for a medical student from the Mercy Medical Center service area in memory of Dr. Bunker.

Selection will be made from all applicants based on community service, ability and achievement and financial situation. All participants

will be notified in writing. Scholarship is for one year; recipients will be eligible to reapply. Decision made in September. Two letters

of reference also required. Application deadline: August 1.

Scott Burns Endowed Scholarship

This scholarship is based on need. The scholarship amount is $1000.00. T he School of Medicine Scholarship Committee will determine

the recipients during the spring semester.

County Medical Society

Contact your County Medical Society secretary about any scholarship and/or loan funds available for medical students. (To be eligible

you may have to become a member of the society.)

Crusade Scholarship Program

Minority students who are members of the United Methodist Church and who are enrolled in an accredited medical school program

are eligible to apply for assistance. Awards range from $1,000 to $3,500 and are based upon academic merit and financial need. The

application deadline is February 1; applications may be obtained from the General Board of Global Ministries, United Methodist

Church, 475 Riverside Drive Room 1470, New York, NY 10115.

Edwin Eddy Scholarship

Two scholarships of $3,000 each will be awarded to first year medical students. Under normal circumstances, the award will continue

into the second year. Each applicant must submit a 1-2 page essay describing how his or her future primary care practice might interface

with patients who suffer from a communication disorder. Selections will be made by the School of Medicine Scholarship Committee.

Students will be notified of deadline in late fall..

Headwaters Medical Society

Goal of this program is to provide $1,500 to incoming or current medical students who demonstrate need and are academically worthy

of the scholarship. Also interested in supporting a student interested in practicing in a rural setting although this is not a necessity. The

student must be from Beltrami, Clearwater or Cass county or from “a small town in Northern Minnesota.”

Hospital Auxiliary

Contact your local hospital to see if they have any scholarship and/or loan funds available for medical students. Check with the secretary

of the auxiliary.

Itasca Memorial Hospital Auxiliary Scholarship

The applicant must be a resident of Itasca County and admitted to medical school. Financial need must be demonstrated. You must

supply a transcript verifying a commitment to health care, two letters of reference from college faculty/staff supporting the commitment,

and a personal statement concerning educational goals, financial status, and any problems associated therewith. Application deadline

is June 15.

Clem Jaunich Education Trust

Lakeview Memorial Hospital Auxiliary Scholarship

Scholarships will be awarded on the basis of need, scholastic achievement, character, and personality. If student fails to complete the

school year in the program for which the award was granted, the auxiliary must be reimbursed for the portion of the year not completed.

A personal interview is required. Once selected, a student must apply for renewal of the scholarship on an annual basis. The grant is

paid directly to the college and applied to tuition or books. Applications are submitted to the Lakeview Memorial Hospital Auxiliary

Scholarship Committee of Lakeview Memorial Hospital Auxiliary not later than May 15th each year. You must be a resident of

Independent School District #834.

National Medical Fellowships

1) Fellowship Program in Academic Medicine for Minority Students--Open to qualified African-American, Mexican-American,

mainland Puerto Rican, and Native American second and third year medical students. Applicants must be nominated by their medical

school dean. Each student works on a specific research project in a major biomedical research laboratory for 8-12 weeks during the

summer or academic year under the mentorship of a leading biomedical scientist. Up to 38 fellowships are awarded annually.

Fellowships are $6,000.

2) Metropolitan Life Foundation Awards-- Open to second and third year minority medical students who attend specific medical schools.

Candidates must demonstrate outstanding academic achievement, leadership and potential for distinguished contributions to medicine,

as well as financial need. Fellowship $2,500.

Nicolett LeSueur County Medical Society

Members of the society wish that the money be directed to a southern Minnesota medical student who gives indication of returning to

a rural area. The School of Medicine Scholarship Committee will determine the recipient of this award during the spring semester. The

scholarship amount is $2,000.

Ben and Jeanne Overman Charitable Trust

First year medical students who demonstrate financial need are eligible for the Overman Charitable Trust Scholarship. The School of

Medicine Scholarship Committee will determine the recipient of this award during the spring semester. Scholarships are in the amount

of $2,000 and are renewable in the second year.

 

John George Ross Endowed Scholarship

Awarded to medical students on the basis of financial need, academics and rural/family practice potential. The School of Medicine

Scholarship Committee will determine the recipients during the spring semester.

Rutledge Memorial Fund

The goal of this program is to award $200 - $2,000 annually to one or more students who is/are permanent Becker County resident(s),

has/have parents who are permanent residents, or is/are former residents for a minimum of six years. If the applicant lives out of Becker

County, he or she must live within a 30 mile radius of Detroit Lakes, Minnesota. Must have completed one quarter of medical school.

The application deadline is December 15.

St. Mary's Auxiliary/Dr. Mark Rathke Memorial Scholarship

The Auxiliary will award two $2500 scholarship to a second year medical student. Students will apply for this scholarship during

the winter of their first year and awards will be made in March. A committee consisting of members of the Auxiliary will determine

Helen N. & Harold B. Shapira Scholarship

One $1,000 award to a medical student in Minnesota. May be renewed with satisfactory academic progress. Must be working in a

curriculum with potential application to patients with diseases of the heart and blood vessels. Applicants chosen on merit by a committee

of American Heart Association, MN affiliate committee of medical and/or scientific professionals. April 1 deadline.

W. C. Stillwell Foundation Scholarship

First year students are eligible for this program and, if selected, will receive it for four years. Amount of aid will be based on the interest

earned on an investment. Fluctuations, therefore, are likely. Home towns must be in one of the following counties: Renville, Lincoln,

Lion, Redwood, Sibley, Brown, Nicollet, Le Sueur, Rice, Pipestone, Murray, Cottonwood, Watonwan, Blue Earth, Waseca, Steele,

Rock, Nobles, Jackson, Martin, Faribault, and Freeborn. Board of Directors will make the decision

based upon academic undergraduate performance, undergraduate faculty-advisory recommendations, financial need while in medical

school and “the basic personal character of the applicant.” If accepted, a yearly report to the Board of Trustees regarding the use of

the scholarship and the degree of progress is made. Financial need is a consideration for selection.

David L. Tilderquist Memorial Medical Library & Scholarship Fund

Scholarships will be awarded to second year students by a committee of the Board of Directors of the fund. Their decision will be based

on need. Recipients are urged to make tax deductible contributions to the fund when they are financially able. You will be considered

for this scholarship if you complete a FAFSA.

Dr. Edward L. Tuohy Medical Scholarship Fund

Scholarships in the amount of $1,200 are available to incoming first year medical students; financial need is a factor. Recipients are

selected by a committee of faculty of the School of Medicine. You will be considered automatically for this scholarship if you complete

a FAFSA.

UMD Scholarship Fund

This scholarship fund is generated by gifts from alumni of the UMD School of Medicine and donors to the Centennial Scholarship. The

scholarships will be awarded to first and second year medical students and recipients will be determined by the School of Medicine

Scholarship Committee during the spring semester of each academic year. The criteria used for selection are academic achievement

and secondarily, financial need.

Harold and Rhea Walder Memorial Scholarship

The award is $1,000-$1,800/year. Applicants must demonstrate financial need, academic achievement and be in their second year of

medical school to be eligible for this award. The award is non-renewable. Recipients will be chosen by the UMD School of Medicine

Scholarship Committee.

* There are several scholarship opportunities that you will be considered for during your first two years of school by the UMD School

of Medicine Scholarship Committee. You must fill out the FAFSA and a scholarship application form which will be given to you at the

beginning of spring semester, in order to be eligible.

SOURCES OF FINANCIAL AID: LOANS

Federal Direct Student Loan Program

The University of Minnesota Duluth, is starting its sixth year with Direct Lending. Direct lending simplifies the loan process, making

it possible for the School to finance, deliver, and be the primary student contact. It eliminates the involvement with lending institutions

and guarantee agencies. These loans are basically the same as the Stafford loan. The difference is that the U.S. Department of Education,

rather than a bank or credit union, is the lender and delivers loan proceeds through the University of Minnesota Duluth. The maximum

interest rate for a Ford Federal Direct Loan is 8.25%. NOTE: an exit interview is required for all second year students with Loan

Collections, Room 129 Darland Administration.

 

Subsidized Ford Federal Direct Loan

The Ford Federal Direct Loan was formerly known as the Robert Stafford Loan (RSL); this is a long term, need-based loan with 0% interest

while in school and a maximum rate of 8.25% during repayment if you are a first-time borrower. For new borrowers repayment begins

six months after graduation. A 3% origination/processing fee is charged.

Loan maximum is $8,500/year; the aggregate limit including undergraduate loans is $65,500. You must file a Free Application for Federal

Student Aid (FAFSA) to be considered for the Ford Federal Direct Loan. Your Federal Direct Loan will be disbursed to you in 3 installments

Unsubsidized Ford Federal Direct Loan

This loan replaces the Supplemental Loan (SLS). Annual limit of $20,000, aggregate limit of $189,000 minus the aggregate limit of

subsidized loans. The interest rate is variable, but the maximum was 8.25%. If you choose to defer interest while you are in school, it

will be capitalized once at repayment. If at all possible it is best to pay interest while in school, see Dina Flaherty for details. Guarantee

and origination fees are the same as the Subsidized Ford Federal Direct loan.

Blue Earth County Medical Society Loan

Four loans are available from the Dr. W. C. Stillwell Foundation for students from “Southern Minnesota.”

Primary Care Loan

A federal loan, interest free while enrolled and during 12 month grace period after receipt of M.D. degree. Interest is 5% (for PCL loans

made after November 4, 1988) during repayment which may be stretched over a 10 year period. The maximum award for first and

second year students is tuition plus $2,500. There is no cumulative maximum amount. Both principal and interest payments are

deferrable during residency. Parental information must be provided on the FAFSA even if you are an “independent” student. You will

automatically be considered for this program if you complete an FAFSA and include parental information. A commitment to primary

care is required. Recipients must schedule an “exit interview” with the Collections Dept. prior to transferring to the Twin Cities Campus.

Lyon-Lincoln County Medical Society Loan Fund

Three (3) $1,000 loans are available. To request a loan you must provide your financial status and the reason(s) you need the loan,

designate what your special interest is in the medical field and send it to the address below. The loan committee will review the

application and grant the loan. Past loans have been granted to students from the Lyon-Lincoln County area. Students interested in

primary care and rural practice preferred.

Minnesota Medical Foundation (MMF)

UMD students receive support from the Family Practice Loan Fund, created in 1975 by the generous contributions of family practitioners

participating in the UMD School of Medicine. The average loan is about $1,000/year. Simple interest at 6% per annum and repayment

of principal begins within thirty (30) days of receipt of the M.D. degree. No interest is accrued until receipt of the M.D. degree or

termination. Five years are allowed for repayment. There are no deferments for this loan. You will be automatically considered for

this program if you complete a FAFSA.

Minnesota Physicians Foundation of the Minnesota Medical Association

Medical Student Loans

The maximum amount available per student is $3,000 per year. You must be a student member of the MMA and be interviewed by members

of the Medical Student Financial Assistance Committee. There is 0% interest while you are a full-time student and 8% interest during

repayment. Repayment begins immediately after you receive your M.D. degree and must be repaid within four years of graduation from

medical school. There must be demonstrated financial need; application forms may be obtained from the MMA.

Ordean Foundation Loan

Interest-free loans ($2,500) from the Ordean Foundation have been made available for students in their first or second year of medicine

at the UMD School of Medicine. First priority will be given to low-income Duluth residents (income below 185% of poverty line) and

second priority to low-income students that plan to practice in Duluth. Following a two year grace period after residency, $1,000 of

the loan will be forgiven for each year that the student practices medicine in Duluth. If the student leaves the area, payments will begin

for that portion of the loan not already forgiven. Likewise, if the student moves to Duluth after beginning loan repayment, future

indebtedness will be forgiven at the rate of $1,000 per year for each year that he/she resides in the area defined as Duluth

Carl D. Perkins Student Loan

Interest-free while you are a full-time student; 5% interest during repayment. However, repayment begins six months after you receive

your M.D. degree. Repayment begins nine months after you receive your M.D. degree for loans made to new borrowers for periods

of enrollment beginning on or after July 1, 1987. Forebearance of payments may be available for up to 3 years after receipt of M.D.

degree. The aggregate limit is $18,000 for medical students including undergraduate amounts. You will automatically be considered

for this program if you complete a FAFSA.

 

Student Educational Loan Fund (SELF)

This is a loan from the MN Higher Education Services Office. Maximum amount of loan/year is $9,000. Principal is deferred while a full

time student. Interest must be paid quarterly. The interest rate as of January 1, 2001 was 8.25% and has been as high as 13%. Note that

the interest rate will vary throughout the life of the loan and that the rate when you first receive the loan may not be what it is when you

repay the loan. The interest rate changes quarterly. All borrowers must have a “credit worthy co-signer” who will be relieved of obligation

to repay the loan only upon your death or permanent disability. To be eligible for this loan as a first year medical student, your cumulative Student Educational Loan Fund (SELF) student loan debt cannot exceed $40,000. Minimum loan is $500. Repayment The Standard Plan requires monthly payment of both principal and interest for up to nine years. These payments start 12 months after you leave school. The Extended Interest Plan provides two more years of monthly interest only, followed by up to seven years of monthly principal and interest payments whichever is least. You must complete the Free Application for Federal Student Aid (FAFSA) before you can be considered for this loan. The UMD Office of Student Financial Aid suggests that students have their co-signer pay the in-school interest rather than pay the interest payments out of their check. The interest is simple unless two interest payments are missed, then it begins to compound. Note: No residency deferment is available. For loan application. University Trust Loan Fund (UTLF)

You may borrow up to $1,000 per academic year from this program, which was established by the University from the Gilfillan Trust

Fund. Interest at the rate of 3% which must be paid while you are a student; may be paid annually. Repayment to the UMD Financial

Collections Office begins the first day of the third month after receipt of your M.D. degree or upon termination of schooling. Interest

during repayment is 6%; minimum monthly repayment is $30. Limited funds available. You will automatically be considered for this

program if you complete an FAFSA.
