Pre-Application Form: Use this form to provide pre-application information related to your interest in an internship.

Final Agency Choices Form: Use this form to provide information about your top two choices for agencies at which to do your internship.

Internship Application for Extension ("Grade of X") Form (.pdf): Use this form to apply for a continuation of your internship beyond the academic term in which you enrolled in Sociology 4597.

Internship Evaluation Form for Students (.pdf): Complete and submit this form upon finishing your internship.

Internship Evaluation form for Supervisors (.pdf): Use this form to give to your supervisor for evaluation.

Request for Waiver of Student Service Fee (.pdf): Complete and submit this form if your internship placement is more than 75 miles from the UMD Campus.
