Forms available in SBE 21:

Degree worksheets (for both Accounting and Business Administration)

Petitions - This form is used for a variety of student requests. Ask a student affairs staff member for specific instructions.

Graduation Plans - these forms are to be filled out 2 terms prior to graduation. A graduation plan hold (RK hold) will be automatically placed on your record after 90 credits to remind you to do this paperwork.

Applications for Candidacy - when you receive a RJ hold, you must come to the office to fill out an application. We use this application to monitor students' eligibility into upper division SBE courses.

Liberal Education Sheets

Change of College Application Form - This applications is used to changed to a different collegiate unit at UMD or to attend a different campus in the UM system.

Senior Checklist

Course Repeat Cards - fill out this form whenever you repeat a course. The card goes to the Campus Center Info Desk.
