PHARMACOKINETIC mechanisms
- Induction of P450 metabolism:

chronic alcohol, barbiturates, carbamazepine, phenytoin (ultimately), rifampin

- Decreased absorption: cholestyramine
PHARMACODYNAMIC mechanisms
- Increased clotting factor/vitamin K concentrations (esp. decreased plasma volume):
- diuretics, vitamin K
- hyperlipidemia, edema, hypothyroidism, nephrotic syndrome
