This is an agreement between the UMD Department of Education and the student named below, who is checking out a Student Teacher Technology Kit. Each kit consists of a laptop computer, digital projector and digital camera, with cases and standard accessories, and having an approximate value of $3,770.

All Users: I acknowledge receipt of the equipment listed above and agree to the following terms of use:
- I will use the equipment only in connection with my student teaching.
- I will not permit any other person to possess the equipment or use it for purposes other than education.
- I shall not sell, lease or otherwise grant anyone rights to the computer or the software.
- I will adhere to the University's Computing Policy and comply with all applicable rules and regulations, including copyright regulations regarding the software, as explained at: http://www.fpd.finop.umn.edu/groups/ppd/documents/policy/acceptable_use.cfm

All Users: I will return the equipment in the same condition in which I received it. If not, I will be responsible for any damages not covered by warranty, or loss of equipment, up to $500 per item. I realize that I will be subject to criminal prosecution or civil liability and late fees if I fail to return the equipment as agreed. I agree to pay all assessments, damage or replacement fees, and any non-warranty repair costs, either directly or through my University financial account.

All Users: To help the Education Department manage this equipment, I will:
- Keep the equipment locked or otherwise secured from theft at all times.
- Never leave the equipment in a car, where temperatures may be extreme.
- Remove my files from the computer when I'm done with it.
- Report any hardware/software problems, and any needed repairs.
- Complete a brief online survey, to share my thoughts and experiences.

Camera Users Only: To protect the privacy of my students and their parents/guardians, I will follow these photography guidelines regarding students and/or their work:
- Before taking photographs, I will consult with my cooperating teacher and the school principal, review parent / guardian "no publish" forms and lists, and consult students in class.
- When taking and using photographs, I will abide by district policies and student / parent / guardian desires.

All Users: THE UNIVERSITY HERBY DISCLAIMS ALL EXPRESS AND IMPLIED WARRANTIES, INCLUDING, WITHOUT LIMITATION, THE IMPLIED WARRANTIES RELATION TO THE EQUIPMENT AND THE SOFTWARE OF MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE. The student agrees to accept the kit items "as is." In no event shall the University be liable to the student or guardian, personal representative, or heirs for an incidental, special, indirect, or consequential damage of whatever nature arising out of any claim, whether in contract, tort, or otherwise, alleging the University's failure to perform its obligations under this agreement or its breach of any duty, common law, or otherwise, owed to the student.

Pick Up and Drop Off Procedures
- Normally, the pick up and drop off of equipment is done in 120 Montague Hall.
- In alternate locations, by special arrangment, equipment can come and go as early as 7:30 a.m. and as late as 10 p.m. on most days of the week.
- Typically, early appointments are in the Education Office, 120 Montague Hall, while late ones are in UMD's Audio Visual Office, 175 Kirby Plaza.
