Immunization Requirements

All students enrolled in a Minnesota college or university for more than one course must provide dates of immunization against measles, rubella, mumps, diphtheria, and tetanus on the required form. Students must have two doses of MMR (measles, mumps, rubella) after their first birthday and have a DT (diphtheria tetanus) booster within ten years of first registering at the University. The Student Immunization Record form (found under the Registrar's Forms heading) must include month and year of each immunization. Immunizations are not required if the student submits a statement signed by a physician showing that:

  • for medical reasons, the student did not receive an immunization; or

  • the student has experienced the natural disease against which the immunization protects; or

  • a laboratory has confirmed the presence of adequate immunity; or

  • the student submits a notarized statement that the student has not been immunized as required because of the students conscientiously held beliefs; or

  • the student was born before 1957.

All undergraduate students, except those graduating from a Minnesota state high school, must complete a Student Immunization Record form, found under the Registrar's Forms heading, (even those with the above exemptions) and return it to the UMD Health Services.
