Forms for the MSW Program

• Add/cancel courses petition after the 10th day of the semester.
  • After obtaining approval from instructor(s) complete Part A & B and the 2nd page.
  • Submit to the Student Support Assistant for routing of signatures.
• Add/cancel courses to the Program Degree Form
  • Complete form following instruction #1
  • Submit to Student Support Assistant for routing of signatures
• Advisor Change Request Form
• Advisor Change Request Form (use if a "Program Degree Form" has been submitted)
• Application for Degree Form
  • All graduate students must submit form to graduate.
  • If planning to walk at commencement (May 13, 2010), deadline to submit form is March 1, 2010 to the UMD Graduate School. NO EXCEPTIONS!!

• Degree Program Form
  • All graduate students must file a program listing of all courses that will be completed and used to satisfy degree requirements. The Graduate School may put a registration hold on student's record once student has reached 25 credits and the form has not been completed and submitted to the Graduate School.
  • Approval of degree program form can take up to 3-4 months.
  • If planning to walk at commencement (May 13, 2010), deadline to submit form is January 15, 2010 to the Student Support Assistant.
