2/13/1999 Minnesota Regional Ambulance Run Report

Airway normal. Breathing normal and labored. Skin normal, moist. Abdomen soft. Chest normal. Capillary refill normal.

Circulation:
16:19 - Blood pressure 150/120. Pulse 150 regular. Respirations 22.
16:30 - Blood pressure 150/110. Pulse 124 regular. Respirations 20.
16:45 - Blood pressure 170/130. Pulse 137 regular. Respirations 20.
16:50 - Blood pressure 170/170. Pulse 113 regular. Respirations 20.
Level of consciousness: Spontaneous.

Verbal response: Orientated.

Motor response: Obeys commands.
Pupils were size 2 and reactive.
C/O pressure in the chest but it is relieved now with O2 high flow nasal cannula. Patient is sweaty and short of breath. Felt a fluttering in his chest that is now gone with O2 but comes back with any movement. Patient is in a sin tac with a few PVC's sometime bigeminal. BP increased. IV attempt x 2. IV's infiltrated left arm. Attempt right arm Successful. Normal blood glucose x 2. Patient pain free and now less short of breath with O2. Second IV site 20 guage. Patient stated he now feels better. Monitor:Ambulance en route sinus tac with bigeminal PVC At hospital sintac with PVC. Given to SJMC pain free.
