Physician's Notes from Emergency Dept

Dr Mergent Chief Complaint: C/P with SOB. Weakness and chest palpitations.

History of Present Illness: Injury. Patient got a sudden onset of C/P with SOB and weakness and felt a flutter and palpitations. Felt like blacking out and called 911. On arrival B/P 150/90 Pulse 90 Resp 20

Past History: Allergies/Meds.: Colostomy repair reversal 3 yrs ago. Kidney stone removal by ultrasound lithotripsy 11 mos ago. Has some hx early COPD 2o smoking. Some Hx anxiety. Was on meds for this. Hypertension. Diabetes. Allergy to clams. Meds are Zestril, Gluconase and pain meds.

Impression: Rule Out M.I. Admit to Hospital. R/O MI

EKG Monitor: Some non specific T wave changes.
