2/13/1999 St. Jennifers Med Center - Dr. Croaker Progress Notes

47 with 5+ minutes of chest tightness with diaphoresis. Diaphoresis has been present on and off all week with gastroenteritis. Symptoms: Emesis and diarrhea - 4 stools already today. UO okay. Felt some ectopy and chest tightness. No radiation of symptoms. No previous cardiac symptoms. He is hypertensive. Quit tobacco 5 years ago (1 pack per day for years prior). Has some early COPD 2o smoking. Some Hx anxiety. Has been on meds for this but not recently. He did feel some ectopy 1 week ago.

Past Medical History: Renal stones, lithrotrypsy recently, high BP, obesity.

Meds: Zesteril 10 mg qd. No known allergies.

Objective: 47-year old NAD - increased BP and tachy . Now settling down. Rare ectopic - paramedics noted bigeminy with exertion at site. Lungs clear. HEENT: Tachy without murmur. 1 ectopic noted and he felt this in 10 min. interview. Abdomen soft, slightly tender lower left quadrant. Bowel sounds within normal limits. No hernia. ECG - NSST and T changes not suggestive of acute MI. Chest x-ray pending. Lytes pending.
Assessment: Chest tightness. Possible ischemic heart disease. Question symptoms secondary to ectopy, secondary to electrolyte imbalance secondary gastro intestinal. ? anxiety?

Plan: R/O MI regular protocol.
